_____________________ is excused from ______________________ class (Block _____) due to completing the requirements for the final. This includes receiving a 95% by May 26th. Both the teacher and parent need to sign this form for the student not to take the final. They may attend class and work on other course work or study for other finals. They are not required to attend my class on final exam day. This permission slip serves as an excused absence form for my class. Underclassmen(freshmen and sophomores) must be signed out at the office. If they do come to school they must be in class during the testing time with other class work or a book to read.
