EXCUSED FROM FINAL EXAM

Print your name, class, and Block. Have Ms. Davis sign this before you take it home. Then have your parents sign and return this by Wednesday, May 25th.

He/She has above a 95% and has completed the project final and does not have to attend class during the scheduled Final time on Thursday or Friday. Both the teacher and parent need to sign this form. They may attend class and work on other course work or study for other finals. They are not required to attend my class on final exam day. If the student is on campus during the scheduled final time, they must report to my classroom. This permission slip serves as an excused absence form for my class.
